ADULT SERVICES

TARGET POPULATION

    Adults over the age of 18 who are being harmed or threatened with harm because of abuse, neglect or exploitation or who are exhibiting gross self-neglect AND who are unable to protect themselves because of impaired decision-making capabilities.

    Homebound, chronically ill elderly and/or disabled adults requiring in-home services which includes, but is not limited to, homemaker/ home health services in order to be maintained in the community.

    SSI recipients in need of emergency assistance because of actual or imminent homelessness.

    Recipients of General Assistance (Consolidated Municipalities) in need of emergency assistance because of actual or imminent homelessness.

    SSI or Medicaid eligible adults in need of transportation services for health reasons.

SPECIALIZED PROGRAMS/SERVICES

1. EMERGENCY ASSISTANCE

    A program for SSI and General Assistance (consolidated towns) eligible individuals who are in a state of actual or imminent homelessness who have had no opportunity to control or plan in advance for the emergency. The assistance provided includes, where necessary: emergency shelter and food, replacement of clothing and furniture, and, when needed the provision of security and utility deposits, and temporary rental assistance to assure a permanent living arrangement.

2. ADULT PROTECTIVE SERVICE

    A program funded through the NJ Department of Health and Senior Services for adults over the age of 18 living in the community who are in a situation of actual or potential abuse, neglect or exploitation or who are grossly self-neglecting. Services include the investigation and assessment of referrals to the program, the direct provision, arrangement and/or purchase of services necessary to ameliorate the situation, the ongoing risk assessment and case management of those adults determined to be in need of protection, and the provision of psychiatric medical and legal consultation services where indicated. 3. HOME HEALTH SERVICES

    A program funded through NJ Department of Health and Senior Services and with Peer Group Initiative Funds which offers the arrangement, purchase, and ongoing case management of homemaker/home health services for elderly and/or disabled adults who require personal care in order to remain in their own homes.

4. COMMUNITY CARE PROGRAM FOR THE ELDERLY AND DISABLED
    (CCPED)

    A Medicaid waiver program offering case management services to elderly and disabled adults at risk of institutionalization who are financially and medically eligible. Case managers arrange home-based or community services designed to maintain individuals in their own home which may include: home health services, medical and social adult day care, medical transportation, homemaker and respite care.

5. MEDICAL TRANSPORTATION SERVICES

    A Medicaid funded program to arrange and purchase transportation services for SSI /Medicaid eligible individuals in need of medical care.

RESIDENTIAL SERVICES

POPULATION SERVICES

1. Residents of rooming houses, boarding homes and residential health care facilities needing protection because of abuse, neglect or exploitation.

2. Residents who need placement in a facility or transfer from one facility to another offering a higher level of care.

3. Residents who need transfer to another facility because they reside in a facility which has been cited for violations and may be closed.

4. Residents requiring services ancillary to those provided by the facility and who do not have the resources or ability to arrange such services on their own.
