community care program
for the elderly and disabled

a program to help eligible individuals to remain in the community, or to return to the community, rather than be cared for in a long term care facility or a hospital setting by use of specific home and community based services . the cost to medicaid for services provided under ccped generally cannot be greater than 70% of the cost of institutionalization of that individual.

model waiver

a program to help eligible individuals to remain in the community or to return to the community rather than be cared for in a long term care facility or a hospital setting by use of home and community based services. the cost to medicaid for services provided to the model waiver client cannot be greater than the cost of institutionalization of that same client. those served under this program would normally not be eligible for medicaid services while living at home because of their own income or because of ssi deeming rules.

new jersey care

a program to help aged, blind, and disabled individuals, and pregnant women and children under age 19 who meet the financial eligibility requirements. this program provides expanded eligibility for medicaid to a group of individuals whose income exceeds medicaid only or tanf/wfnj standards. aged, blind and disabled individuals must have income below the federal poverty guidelines. eligibility for pregnant women and infants up to age one is established when family income does not exceed 185% of the federal poverty guidelines. eligibility for children under age 19 is established when family income does not exceed 133% of the federal poverty guidelines.
