IMPORTANT NOTICE FOR ISSUING PAYMENTS

Pursuant to IRS regulations and Bergen County Board of Social Services procedure, we require proper documentation to certify your official identity with the Internal Revenue Service.

INDIVIDUALS AS LANDLORDS:

Please complete Form W-9 as per IRS instructions.  In addition, we require a legible copy of a Social Security card and proof of ownership of the property which matches the information contained on Form W-9.

BUSINESSES:

Please complete Form W-9 and provide acceptable documentation from IRS showing the tax ID number and business name.  Additionally, a copy of the New Jersey Business Registration certificate is required.

Please note that any missing or illegible material may delay or prevent payment processing as these materials must be received before any payment is released from this agency.
